This can only be obtained from a licensed medical examiner as listed by the Federal Motor Carrier Safety Administration (FMCSA) National Registry. You’ll also need to bring along a valid current driver’s license and proof of citizenship or permanent legal residency. For this last piece, a green card, social security card or birth certificate will do nicely. Then you’ll be all set to make your first visit a successful one.
